Cancer patients are surviving at increasing rates today , but successful treatment in younger patients with chemo and radiation often leads to reduced fertility potential . Chemotherapy also is often used for noncancerous conditions such as autoimmune diseases like systemic lupus erythematosus (SLE) and hematological diseases. If damage to reproductive organs from treatment is likely, cryopreserving gametes, embryos, or gonadal tissue may help to preserve fertility.
Techniques for freezing sperm and embryos are well established. Techniques for oocyte cryopreservation have seen dramatic improvement in the last decade with improved pregnancy outcomes; however, long–term data on outcome are still limited. Techniques for freezing testicular and ovarian tissue are still experimental. Five–year survival rates with testicular cancer, hematologic malignancies, breast cancer, and other cancers that strike young people may be in the 90% to 95% range. However, treatment of these cancers is often highly detrimental to both male and female reproductive function.
The testis is highly susceptible to the toxic effects of radiation and chemotherapy at all stages of life. Cytotoxic chemotherapy and radiotherapy may produce long–lasting or persistent damage to primordial sperm cells, leading to oligo– or azoospermia. The most common strategy to preserve fertility is cryopreservation of sperm before treatment for later use. Cryopreservation of testicular tissue from prepubescent males remains experimental.
Ovarian damage is drug and dose dependent and is related to age at the time of treatment, with progressively smaller doses producing ovarian failure as the patient’s age increases. Total body, abdominal, or pelvic irradiation may cause ovarian and uterine damage, depending on radiation dose, fractionation schedule, and age at time of treatment. An elevated serum follicle–stimulating hormone (FSH) level is the most commonly used biochemical indicator of ovarian damage and failure. However, Anti Mullerian Hormone (AMH) and Antral Follicle Count (AFC) are emerging as other markers of ovarian aging. Preservation of fertility in females is more complicated than in males. Conservative fertility–sparing treatment such as radical trachelectomy in cervical cancer, hormonal treatment of early endometrial cancer, and conservative surgical management of early–stage epithelial ovarian cancer may be possible for certain women with early invasive disease. Reducing the radiation dose to the ovary by shielding or surgically removing the ovaries from the field of radiation (i.e., oophoropexy) may preserve ovarian function. Suppression of folliculogenesis with GnRH as for fertility preservation has long been controversial. If the cancer treatment can be delayed, it is possible to undergo ovarian stimulation and retrieve and freeze eggs (both mature and immature) or produce embryos that can be frozen for later transfer to the individual or a gestational carrier. Ovarian tissue freezing prior to the initiation of gonadotoxic treatment is still experimental with several live births reported worldwide. It is becoming a viable option for prepubertal girls where oocyte and embryo freezing is not an option, for women who either cannot delay treatment or hormonal treatments are contraindicated, or for women undergoing risk reduction salpingo–oophorectomy.
Approximately, 10–15% couples of reproductive age group seek fertility assessment at various clinics worldwide. With an increasing population of working women and the postponement in the ages of marriage and initial childbearing, infertility services are being increasingly asked for. With the advent of assisted reproductive techniques (ART) and advances in embryology the evaluation of the male partner is often overlooked which is responsible for approximately half of the infertility cases. It is essential to identify the pathology and treat the male partner, which may allow couples to improve their fertility potential and conceive naturally too.
Success of intracytoplasmic sperm injection (ICSI) had led to the cessation of research in sperm dysfunction and allowed the indiscriminate use of immature as well as defective sperms.
The new World Health Organization (WHO) guidelines on semen analysis are enthusing and makes one wonder whether we have over treated the male partners previously. The oligo-astheno-teratospermia syndrome is commonly encountered a problem in male infertility. This creates a challenging situation for the andrologists as this is a treatable condition if diagnosed at the correct time and treated accordingly. Understanding the human sperm and fertilization biochemistry has improved over the years. Objective data and information about important measures like kinematics of sperm capacitation, hyperactivation, the ability of sperm to bind zona-pellucida, undergo acrosome reaction, and finally decondensation of the chromatin to form male pronucleus started coming in to improve our knowledge of events leading to fertilization and formation of embryo. Such recent information is helping us to diagnose new forms of male sub-fertility, predict the success of attempts at natural or assisted conception and to design in vitro sperm function tests. This is guiding us to design new treatments to overcome the diagnosed dysfunction.
Preimplantation Genetic Diagnosis (PGD) is a relatively recent diagnostic modality for identification of genetic defects in embryos generated through In Vitro Fertilization (IVF), prior to implantation in the mother’s womb. Thus, PGD facilitates birth of healthy babies in couples where both the parents are carriers of genetic aberrations, and hence, at risk of transmitting heritable diseases to the offsprings. In addition, this technology can be utilized in the field of assisted reproduction for aneuploidy screening and diagnosis of unbalanced inheritance of chromosome abnormalities, such as translocations or inversions. Thus, PGD identifies the genetic aberrations at the preimplantation stage by negative selection of the affected embryos.
Indications for PGD are detection of specific genetic diseases in couples identified with both the partners as carriers of genetic aberrations including autosomal recessive, autosomal dominant, X-linked monogenic disorders and chromosomal structural aberrations such as balanced translocation. PGD facilitates identification of genetic aberrations at genomic or chromosomal level in a single cell of the embryos at the pre-implantation stage. PGD provides the couples a window of opportunity to test the embryos for genetic diseases identified in the carrier stages and provides clinical guidance in an ongoing pregnancy. In addition, a variant to PGD is preimplantation genetic screening (PGS) whereby embryos are screened for numerical chromosomal aberrations in a single or both partners with normal karyotype, experiencing poor reproductive outcome including advanced maternal age, repeated IVF failures, repeated spontaneous abortions, severe male factor infertility and unexplained infertility. The patient populations are at risk of failure with IVF because of a high proportion of aneuploid embryos. PGD is believed to decrease the risk by selecting chromosomally normal embryos that have a higher chance of success post-implantation.
The current trend is towards the use of more advanced technologies like whole genome amplification and array based technology, next generation sequencing etc. in PGD for various genetic disorders. Also for preimplantation genetic screening where cleavage stage embryo biopsy and fluorescence in situ hybridization were the method of choice, the evidence has suggested that there is not advantage of PGS using this strategy. Hence, the focus is now migrating to the use of polar body biopsy and trophectoderm biopsy along with array and NGS based technology for testing.

Lady Hardinge Medical College Safe motherhood encompasses a series of initiatives, practices, protocols and service delivery guidelines designed to ensure that women receive high-quality gynecological, family planning, prenatal, delivery and postpartum care, in order to achieve optimal health for the mother, fetus and infant during pregnancy, childbirth and postpartum.
Millions of women worldwide are affected by maternal mortality and morbidity from preventable causes. Adequate delivery assistance by a skilled person without excessive recourse to cesarean section is an international commitment to improve maternal health. Care for obstetric emergencies with adequate referral to appropriate facility starting from grass root level is also a part of this. After decades of planning India is still at 167 maternal deaths per 100000 live births, Sample registration survey, 2015.
In this workshop on safe motherhood aim has been to address practical management problems of day to day obstetric practice with an aim to enhance skills and to learn latest guidelines by stalwarts in their fields.
Hemodynamic of pregnant women differs from a non pregnant one and it is important to identify these differences. Session on Critically ill pregnant patient addresses this along with severe sepsis and breathlessness in preeclampsia. Pulmonary edema in preeclampsia is an important cause of near miss situations and ICU admissions. Termination of pregnancy becomes imperative and is an obstetric dilemma.
Massive hemorrhage is the leading cause of maternal mortality and morbidity world over. Transfusion of appropriate components at right time, arrest of hemorrhage with quick decisions whether it is devascualrisation or tamponade or a timely decided obstetric hysterectomy is critical in saving maternal life. Session on massive obstetric hemorrhage addresses all these issues.
Rising cesarean rate and their long term complications especially adherent placenta is every one’s concern. Recent guidelines addressing safe prevention of primary cesarean delivery is an important session. It aims to discuss important common indications for primary cesarean like failed induction of labor, prolonged labor, breech and twins and emphasizes which are the situations which merit a cesarean delivery.
Differentiating and understanding an abnormal trace from normal or an insignificant change from ominous is very important to manage high risk pregnancies. Different case scenarios with different CTG traces will go a long way to make the delegates understand fetal distress.
Skill enhancement is important to hone up skills in emergency obstetric care. Videos of dealing with adherent bladder, broad ligament hematoma and morbidly adherent placenta will give an insight in managing these difficult situations with competence.
And lastly hands on workshop is important for each and every postgraduate and obstetrician to give a visual and hands on experience. Assessment of blood loss which is normally always underestimated will be shown. Various procedures to estimate blood loss like visual estimation, measuring and weighing gauzes biochemical assessment is important to learn. Internal compressions and how to place a condom catheter is most important procedure to learn while managing postpartum hemorrhage. PPH drill and medical management of post partum hemorrhage is important so that right drugs are given at right time in right doses.

Denial to a girl child of her right to live is one of the heinous violations of right to life. Gender bias and deep rooted prejudice and discrimination against girl child and preference of male child have led to large scale female foeticide in last decade. Declining sex ratio of girls and female in India is a major concern. Female foeticide or sex selective abortion is the elimination of female foetus in womb. Sex of the foetus is determined by Amniocentesis/CVS/USG.
In order to check female foeticide, PNDT ACT 1994 was enacted and brought into operation from 1st Jan 1996. Rules have also been framed under the Act . The Act prohibits any advertisement relating to prenatal determination of sex and persuades punishment for its violation in terms of imprisonment and fine. Urodynamics encompasses all of the diagnostic modalities used in the evaluation of bladder and urethral function. It includes scientific observations and measurements to enhance understanding of how the lower urinary tract works.
Many of the health professionals interested in the field of lower urinary tract disorders are not able to utilize this modality due to lack of their familiarity with the equipment, tracings and inability to correlate the results with their clinical findings. Usually they are dependent on other specialists for conduction of test and interpretation, hence they never feel confident in applying this knowledge in their clinical practise with a hidden fear of getting biased by someone else opinion, who has not seen the patient.
Therefore we have organized this one-day workshop, which is designed to provide an educational opportunity for urodynamic examiners who seek assistance in interpreting urodynamic testing results and applying this to their practice. Detailed knowledge of equipment and method of performing this test will make them aware of recognizing the artifacts and pitfalls so that any misinterpretation can be avoided.
At the conclusion of the course, participants will be able to run and interpret basic urodynamic examinations of high quality. This course will also help them to understand why urodynamics is helpful, to improve diagnostic skills and to learn when and how to refer after urodynamics.
IN THIS WORKSHOP YOU WILL LEARN:
Urologists, gynaecologists, technicians, nurses and other health care professionals.
This course will give an up-to-date perspective of understanding Urodynamics, covering both theoretical and practical aspects.ing objectives will be:
The highlights of this workshop include lectures regarding the basic urodynamics – components & key consideration and update on urodynamic practice & standards. We would be discussing indications of Urodynamics and systematic approach to interpretation of standard traces and writing a report. Presence of renowned experts will give us an opportunity to further unfold the saga of knowledge and solve our queries in “Case based discussions” session. During video demonstration session we will have practical demonstration of setting up an urodynamics unit and recognition and correction of artifacts by applying basic troubleshooting techniques. This workshop has been exclusively planned keeping in view the interests of both Urologist and Gynecologist, as a key person encountering these disorders in their routine practice.
